Default address keeps changing to :fail:

Over the last month on at least two occasions my default email address has been changed to :fail: from the one I set. This is unacceptable. I am unsure of how much email I have lost due to this. Is this a new automated feature of our email software or is someone hacking my account?

The default address can only be set via cPanel. Ultimately we do not recommend using it at all, it does nothing but attract spam and can even cause performance problems on the server when spammers target an entire domain. You should create e-mail accounts and forwarders for just the addresses that you need to use.
