I set up a reply form on one of the sites and did everything by the book.
Finally it all worked until I added "required" fields.

In Firefox it all works fine.
Using IE, whenever there is a "required" field missing, the browser returns a "PAGE NOT FOUND" message.

Any ideas?

Are you using full page URL's to send the user back for empty fields?
Have you looked at the error log under your cPanel login?
