Can't find the survey...

Hello everybody.
I suspect it's due to me trying to do such stuff before the first morning coffee, but I just can't find where the link to the Dathorn survey is. Should I try changing themes?

Customer Control Panel. You know that place where you submit troubles tickets and always wind up having to look up your username and password because you use it so seldom.
